Mission Statement The mission of the Pleasant
Acres Nursing & Rehab Center is to provide the citizens of York County
who need nursing home care with a long term care facility that maintains
the highest level of resident care available and encourages each resident
to achieve the fullest use of his mental, physical, spiritual and social
capacities. Admission is available to all adults without regard of race, creed,
color, sex, age, handicap or national origin. All residents shall be treated with the care that will ensure
their personal dignity. We feel that caring about people is as important as caring for
them. Therefore, kindness and
understanding shall be afforded all residents and their families by a team
of caregivers that abide by the highest professional standards.
Auspices
On February 6, 1804, the state legislature passed an Act which granted the York County Commissioners power to levy taxes for the purpose of purchasing land and erecting a building to house and care for the poor of York County. Today, the Pleasant Acres Nursing & Rehab Center, formerly the York County Nursing Home, is a licensed long-term care facility with the capacity to care for 375 residents. The facility is licensed for operation by the State Department of Health and regulated by a number of state and federal agencies.

Purpose
The primary objective of the Pleasant Acres Nursing & Rehab Center is to provide the citizens of York County who need nursing home care with a long term care facility that maintains the highest level of resident care available and encourages each resident to achieve the fullest use of his/her mental, physical, spiritual and social capabilities.
Services
Nursing services are provided by a trained staff of professional registered nurses, licensed practical nurses and nursing assistants under the leadership of the director of the nursing services. Based on an evaluation of the medical, physical, social and emotional needs of a resident, an individual nursing program is planned for each resident.
A complete physical examination is performed on each resident within 48 hours of admission. Physicians are available on a twenty-four hour a day basis, seven days a week under the supervision of the medical director.
A special unit is available for chronic ventilator dependent residents and a licensed team of respiratory therapists provides care to ventilator and respiratory compromised residents requiring tracheotomy care.
An attending physician prescribes a therapeutic diet for each resident. Under the supervision of a qualified food service director and a registered dietician, the dietary department prepares meals that meet the daily nutritional needs of residents.
Meals are served in the dining room located on each nursing unit. Residents who are able are encouraged to eat their meals in the dining room. Efforts are made to encourage the residents to socialize during their meal period to make this time of the day a pleasant one.
PANRC operates a fully equipped laundry facility, which processes bed and bath linens as well as the resident’s personal clothing. Arrangements can be made for the family of the resident to do resident’s personal laundry if so desired.
A licensed pharmacy operates in the facility under the direction of a licensed pharmacist. Medication is dispensed from the pharmacy upon orders of the staff physicians. All medication is administered to residents by licensed nursing staff.
The Environmental Services and Plant Operations departments are responsible for providing a clean, safe, sanitary and pleasant environment for both the residents and the employees.
The Therapeutic Recreation Department plans daily activities to meet the residents’ physical, emotional and social needs. Every resident has the opportunity to participate in recreation programs designed to enhance and improve his/her quality of life. Throughout the year, a number of special events are scheduled in which families may participate.
Pleasant Acres Nursing & Rehab Center maintains both a barber and a beautician service area. A barber and beauticians employed by PANRC provide both in the service areas and on the nursing units.
A full-time chaplain serves as liaison between PANRC and the pastors or rabbis of residents. Ecumenical services are held in the multi-purpose room on Sunday afternoons and evenings. Catholic Mass is held monthly. A Eucharist minister and brother from St. Joseph’s Parish visit weekly. Bedside communion is offered by the chaplain quarterly to residents unable to attend services.
The Social Services Department identifies and addresses the social and emotional needs of the resident and assists each resident in adjusting to his surroundings and in the understanding and accepting of his illness or handicap. Support and counseling is also available for the family of the resident. The caseworkers assist with the development of a discharge plan for each resident and make recommendations on the utilization of community resources.
Comprehensive dental services are provided in the facility on a weekly basis.
Optometry services are available in the facility on a weekly basis.
Physical/Occupational & Speech Therapy
Physical therapy (PT), occupational therapy (OT) and speech therapy (ST) services are provided both in the Rehabilitation Services Department and on the nursing units under the direction of licensed therapists. Therapy is initiated after receiving orders from the resident’s attending physician.
Podiatry services are available in the facility on a weekly basis.
Volunteers extend the work done by Pleasant Acres Nursing & Rehab Center staff and enrich the lives of residents. Examples of volunteer work within the facility include, but are not limited to: sorting and delivering residents’ mail, operation of a clothing store and “Country Store,” transportation of residents within the facility, assisting with various activities, visit with residents, acquainting new residents with the facility, escorting residents on outings in the community, writing letters for residents and reading to residents.
1. Application process is initiated through the Admissions Department.
2. Applications must have documented medical and psycho/social needs for facility services as certified by a physician.
3. All applicants who will apply for Medicaid payment will require a level of care assessment completed by the OPTIONS II Department of the York County Area Agency on Aging prior to admission.
4. Applicants who have been a resident of the County of York, Pennsylvania, for at least 12 months prior to the date of admission are given priority.
5. Applicants with a primary diagnosis of mental illness or mental retardation will not be considered eligible for admission without an appropriate State exception for nursing home placement.
6. Ambulatory alcoholics will not be accepted for admission.
7. PANRC shall admit only those applicants whose nursing care and physical needs can be met within the facility.
