HUMAN SERVICES DEVELOPMENT FUND (HSDF)

Background
The Human Services Development Fund was created “for the purpose of encouraging county governments to provide locally identified services that will meet the human service needs of citizens in their counties “[Act 1994-78 Section 4]. The fund “shall be used by the county solely to provide and administer county based social services and service coordination within the county “[Act 1994-78, Section 6 (b)]. The Human Services Development Fund (HSDF) was implemented in fiscal year 1984-1985 at a statewide funding level of $4 million. In 1985-1986, ASBG AND HSDF were merged into the same funding stream. In 1994, HSDF was enacted into law by Act 78.
HSDF funds afford counties the opportunity to develop services tailored to meet particular needs that may not be met within the categorical fund structure, to develop processes that are intended to enhance the coordination of activities among human services agencies and programs, and to expand existing categorical services within the county programs. This is the only source from the Commonwealth available to fund services for low-income adults. There is no county match required. Target Population
The target population consists of program-eligible consumers whose needs are not being adequately met through the current categorical funding stream, either through lack of sufficient funds, programs or uniqueness of service needs.
Funding is directed to low-income adults between the ages of 18 and 59. HSDF funds are used in the County Human Service system to provide services in the following areas:

PRE-VOCATIONAL SERVICES
Serves MR consumers on the waiting list. Services provide developmental and vocational training within a community-based facility (sheltered workshop) through the use of subcontracted work from local business and industry.
B. CATEGORICAL SERVICES, Community Based
Counties may use HSDF funds for services to fund categorical programs, as defined by the state, that are specific population groups. The services described below are administered by Providers in the community. Eligible programs include the following:

Consumer Eligibility – Community Administered Categorical Services:
· Service is for adults who meet a low-income criteria [varies by service] and also an age criteria [ages 18-59].
· Eligibility is determined every six months and requires a completion of a standard HSDF form.
¨ Community Based: CASE MANAGEMENT
Services involve a series of coordinated staff activities with consumers to determine the services that are needed. Staff then provide coordination for the timely provision of those services. Service activities include assessment, exploration of service needs, development of service plans, arrangement for prompt delivery of required service, liaison with providers, provision of personal advocacy and follow-up to consumers. Case Management is provided by utilizing neighborhood centers throughout the County.

¨ Community Based: EMERGENCY SHELTER SERVICES
Emergency shelter services are provided for families and women who are homeless or in need of emergency housing. Length of stay will vary from overnight up to thirty (30) days.

Community
Based: HOMEMAKER SERVICE
Provides home help, personal care, respite services and instructional services. The services are provided in the consumers’ homes by trained, supervised homemakers. Eligible persons may be as young as 18.

This service provides intensive housing counseling and case planning services to assist consumers to attain and/or retain stable housing.

Generic Services are “services provided to meet the needs of two or more client populations.” The ten Generic Services are: Adult Day Care, Adult Placement, Centralized Information and Referral, Chore Services, Counseling, Employment, Homemaker, Life Skills Education, Service Planning/Case Management and Transportation Services.
Consumer Eligibility--Generic Services:
· Applicants must either meet the eligibility requirements for at least one of the categorical programs included under the Generic Services; or
· Applicants must meet the financial eligibility requirements of Adult Services found in 55 PA Code, Chapter 2050, Eligibility for Adult Services Funded through the Adult Services Block Grant; also
· The County must establish a need-criteria appropriate to the service being offered which is approved by the Department of Public Welfare.
· For generic or specialized services, the county may choose to develop a sliding fee scale.
¨ Generic Service: INFORMATION AND REFERRAL SERVICES
This service links people in need with the appropriate agency or service designed to eliminate or alleviate that need. It includes a brief assessment of the problem, provision of relevant information and referral to community resources, follow-up as appropriate, maintenance of accurate up-to-date information and systematic data collection. Residents who speak neither English or Spanish can be served by FIRST through their use of a multi-language translation service.

Provides transportation services to financially and age eligible persons.

Defined as “new services or a combination of services designed to meet the unique needs of a client population that are unmet by the current categorical programs.” Specialized Services provide the county with flexibility to provide innovative services that address unique county needs.
Our community benefits from a broad range of community services providing a Continuum of Care to meet resident’s needs. Yet, there remain areas of need that could be developed under this category, contingent on adequate funding.
Consumer Eligibility – Specialized Services
· Applicants must meet the categorical program eligibility requirements of programs included in the Specialized Service; or
· Applicants must meet the financial eligibility requirements of Adult Services and need criteria outlined under Generic Services; and
· For Generic or Specialized Services, the County may choose to develop a sliding fee scale.
Specialized Services:
None currently funded in this area due to funding limitations.
Service Coordination is defined as “planning and management activities designed to improve the effectiveness of county human services.” The goal is to improve coordination, planning, management and service delivery.
Community-based Consumer Access
Increasing community awareness, resident accessibility and participation in services are goals supported by HSDF. The initiative that encompasses these goals and therefore benefits from HSDF support is Family Service System Reform (FSSR).
