            Regular Absentee Ballot Application

            (Use only up until 5:00pm the Tuesday before each election.)

             The completed document goes to the York County Election Office.

 

            Emergency Absentee Ballot Application [THIS MUST BE NOTARIZED]

(Use for emergencies that happen after 5:00pm the Tuesday before each  

 election until 5:00pm the Friday before each election.)

             The completed document goes to the York County Election Office.

 

            Emergency Absentee Ballot Application [THIS MUST BE NOTARIZED]

(Use for emergencies that happen after 5:00pm the Friday before each

 election until 8:00pm on election day.)

             The completed document goes to the York County Court of Common Pleas.

 

            Note:

-         Designation of Authorized Representative

(This is to be completed by the voter if someone

 will assist the voter in getting an emergency absentee ballot

 after 5:00 pm the Friday before each election.)

 

-         Certification of Authorized Representative

(This is to be completed by the person who has

 agreed to assist the voter in getting an emergency absentee

 ballot after 5:00 pm the Friday before each election.)

 

-         Statement of Absentee Elector Requiring Assistance

(This is to be completed by the voter if someone will

assist the voter to mark (vote) the actual absentee ballot.

This form must be returned with the voted ballot.)

(This is to be completed by the person who agreed

to assist the voter to mark (vote) the actual absentee ballot.
